How You'll Make an Impact

• Be the Guide: Proactively identify and welcome new clients, conducting thoughtful assessments to deeply understand their unique needs and ensuring they stay connected to the care they deserve.
• Offer Compassionate Support: Beyond medical logistics, provide the "soft" side of care—offering informal counseling, health education, and the steady social support that builds true resilience.
• Break Down Barriers: Roll up your sleeves to help clients navigate the system—whether that's scheduling vital appointments, attending visits as a supportive companion, or simplifying the often-overwhelming medical application process.
• Build a Village: Be the glue that connects our department with vital community partners and referral sources, fostering the strong, collaborative relationships necessary for holistic health.

Requirements

• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
