Bilingual LPN Care Coach - Bonus Eligible

About the role

CircleLink Health is seeking a passionate, tech-savvy LPN Care Coach to work remotely and support patients enrolled in Medicare's Chronic Care Management Program. This part-time role involves approximately 20-25 hours per week, focusing on monthly calls with patients to provide education, coordinate care, and promote self-management strategies.

Responsibilities

• Utilize specialized care management software to contact Medicare patients with multiple chronic conditions (such as diabetes, CHF, chronic pain, COPD, etc.) monthly.
• Build and maintain rapport with patients to assist them in achieving better health through SMART goals and education on self-management strategies.
• Implement and improve the Plan of Care by updating medications, appointments, biometrics, symptoms, and interventions.
• Connect patients with community resources such as transportation, personal care needs, prescription assistance, social services, etc.
• Conduct Transitional Care Management activities for high-risk patients discharged from hospitals or emergency rooms to reduce unnecessary readmissions.
• Close care gaps by encouraging and assisting with preventive care measures like annual well visits, vaccines, cancer screenings, specialist appointments, etc.

Requirements

• Fluent in both English and Spanish.
• Able to work independently with minimal supervision while meeting performance metrics.
• Passionate about nursing and improving patient outcomes.
• Proficient with technology and eager to learn new software.
• Excellent organizational and time management skills.
• Timely communication skills, responding to all messages and emails within 24-48 hours.
• Strong critical thinking and problem-solving skills.

Education and Experience

• Current, unrestricted Multistate LPN or New Mexico LPN license required.
• Proficiency with EPIC electronic health record and web-based applications.
• 3-5+ years' experience as a nurse preferred.
• Case Management or Chronic Disease Management experience highly preferred.
• Certified Diabetes Educator experience preferred.
• Experience with Motivational Interviewing or other behavior change communication techniques preferred.

Scheduling and Other Requirements

• LPN must have a strong internet-connected computer and agree to its testing.
• Must have a minimum of 20 hours of availability per week between 8 am and 5 pm MST, Monday-Saturday.
• Schedules are flexible and determined using the company’s software.
• This is a 1099 contract position with no end date.
• Care coaches are responsible for their own equipment, taxes, and insurance.

Benefits

• Compensation: $11.00 for the first 20-minute increment, $9.50 for the second, and $9.00 for the third.
• Monthly payments via direct deposit, 40 days after the last day of the first month of service.
• Bonus eligibility: $300 performance bonus for coaches who successfully complete three months of employment and meet all performance expectations.
