Your Mission in Action
The Medical Coordinator arranges and coordinates appropriate health services, including urgent, emergent, and preventative health services, for children in care.
- Maintain complete, confidential medical records for children
- Ensure appropriate communication of health needs to case managers and clinicians
- Schedule health care appointments
- Cooky transportation to health care appointments
- Ensure qualified interpretation and translation services are in place
- Serve as a liaison for community health care providers
Responsibilities
- Coordinate initial medical screenings with primary care provider within ORR timeframes
- Coordinate routine health screenings and assessments
- Administer prescribed medication and treatment
- Coordinate medical appointments
- Manage nutritional needs to ensure timely and comprehensive care for each UC
- Support infection-control policies and procedures as well as conduct staff training on medication dispensing and Psychotropic medications
- Facilitate trainings for staff, such as dispensing prescribed, over-the-counter and psychotropic medication, bloodborne pathogens, and CPR/Fir st Aid
- Work closely with the Program Leadership and staff to ensure UC are receiving the highest quality of care and the program is operating efficiently and effectively, and is meeting the goals and objectives of the program and the needs of the UC
- Other duties and special projects as assigned
Qualifications
- Current LPN/LVN license in Texas or higher hea l t h –related qualification (e.g. licensed resisted nurse (RN) with bachelor’s degree)
- OR Bachelor’s degree in relevant field
- One (1) year of employment in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical care
- High School diploma or equivalent
- Bilingual English and Spanish, (speak, read and write)
- Must be 21 years of age or older per licensing requirements
- Strong interpersonal, customer service, and relationship management skills
- Ability to build trust with UC and internal stakeholders
- Strong documentation, time management, organizational, and conflict resolution skills, with proficiency in Microsoft Office and UC information systems

Physical Demands & Work Conditions
- This position requires sitting and looking and using a computer for long periods of time
- Position works in a child facility and has constant exposure to children
- Requires frequent standing, walking and time in units/cottages which are area where children reside
- Lift, push, pull, move up to 125 lbs
