Responsibilities
- Serves as the primary point of escalation for case management staff on complex client, family, or agency issues.
- Develops and implements treatment plans and assessments in EHR and UAC Portal.
- Reviews and quality-checks service plans, and reports completed by case management staff prior to submission, ensuring clinical soundness and compliance with contractual/licensing standards.
- Provides weekly individual counseling and related documentation, and maintains a modified caseload as assigned.
- Participates in, coordinates, and communicates pertinent client information at weekly and program staffing for their caseload.
- Writes progress/court reports and discharge summaries as necessary.
- Maintains regular contact and communicates effectively with juvenile probation officers/case workers, including them in the treatment process and updating them on client progress.
- Maintains regular contact and communicates effectively with parents/sponsors/legal guardians.
- Maintains client files in accordance with contractual and licensing requirements.
- Conducts periodic file audits for the case management team to identify documentation gaps and ensure audit-readiness ahead of ORR/licensing reviews.
- Participates in and helps lead team meetings, including case management-specific staff meetings.
- Provides input on case management staff performance, including onboarding, training, and coaching of new or struggling case managers.
- Serves as a liaison between case management staff and the Program Manager/APM on systemic issues, training needs, and process improvements.
- Ensures clients are prepared for transport to court.
- Ensures accurate billing and statistical reporting for clients on caseload, and monitors billing/statistical accuracy across the case management team, flagging discrepancies before submission.
- Ensures communication of pertinent information to outside agencies to assist in providing basic needs for clients, such as IEPs, prior drug abuse or mental health history.
- Completes client satisfaction index at time of discharge.
- Completes referrals and intake processes, and provides guidance to case management staff on complex or time-sensitive intake/referral situations.
- Assists with client behaviors and unit operations as needed.
- Provides coverage for the Program Manager or APM on case-management-related matters in their absence, as assigned.
- Assists in training new case management staff on documentation standards, treatment planning, and Abraxas policy.
- Supports the Abraxas philosophy and mission and promotes the Seven Key Principles of care, modeling and reinforcing these principles among case management staff.
- Demonstrates appropriate use of Safe Crisis Management techniques and skills.
Qualifications
- Education and Experience Required: Associate degree or sixty (60) credit hours from an accredited college or university. Six months’ experience preferred.
- Other Qualifications: At least twenty-one (21) years of age Must have a valid Driver's License Physical exams that include TB and drug testing. Must pass both federal and state background checks which screen for crimes including, but not limited to drug related crimes, violent crimes, sexual abuse, domestic violence, child abuse, and repeated misdemeanors. Satisfactory completion of background screening and applicable pre-employment checks, including but not limited to employment and/or personal reference and driving records. Ability to work with computers and the necessary software typically used by the department.
- Schedule: Full-Time • Shift: Monday – Friday (7:00am-4:00pm) Must be available to work weekends!
