About the role

The Lead Care Coordinator (LCC) position is part of the Enhanced Care Management (ECM) Program. The LCC oversees specific cases, coordinates health care benefits, provides education, and facilitates client access to care.

Responsibilities

  • Assesses client needs in various domains including physical health, mental health, substance use disorder, oral health, trauma-informed care, social supports, housing, vocational/employment, wellness, and referrals.
  • Develops and implements Individual Care Plans/Health Action Plans.
  • Provides services in various settings including office-based, telehealth, or field-based services.
  • Connects clients to additional social services and supports as needed.
  • Coordinates and advocates for clients with healthcare professionals with consent and authorization.
  • Conducts outreach and engagement activities through phone calls, mailed information, and field visits.
  • Coordinates transportation for clients and accompanies them to office visits, as needed and appropriate.
  • Evaluates progress and updates goals.
  • Completes required documentation within established timelines.
  • Attends weekly staff/team meetings and supervision.
  • Attends training as assigned.

Requirements

  • Three years working experience in Health, Education, Social Services, or related field; or a combination of at least one year of work experience and an Associate's Degree or higher; or one year of work experience and two years of lived experience and/or community engagement.
  • A High School Diploma or equivalent required.
  • Bilingual in Spanish/English required.
  • Excellent customer/employee relations and communication skills, including writing, vocabulary, grammar, spelling, and punctuation.
  • Proficiency and knowledge of PC hardware/software operations and applications including Microsoft Word and Excel.
  • Ability to complete training program and ongoing educational requirements as assigned.

Position/Program Requirements

  • Valid California driver's license, personal automobile insurance, and driving record meeting Agency standards.
  • Physical and mental fitness meeting Agency standards.
  • Tuberculosis (TB) test and drug screening test.
  • Personal background investigation conducted by the State of California.
  • Culturally integrated workplace and respect for human differences based on protected classes.

Physical Requirements

  • Adequate range of body motion and mobility to work in an office or outdoor environment including standing and walking (even and uneven surfaces), alternating between standing and sitting for extended periods of time, bending, kneeling, twisting, reaching, balancing, and occasional carrying and lifting up to 25 pounds occasionally, and up to 10 pounds frequently.
  • Physically able to walk up and down stairs routinely.
  • Requires the ability to utilize computer and telephone systems, which includes the coordination of eye and hand and fine manipulation by the hands and fingers for typing, writing, and working with files.

Desired Qualifications

  • Prior experience as a Community Health Worker, Peer Support Specialist, or Medical Assistant or similar role.
  • Prior experience utilizing evidence-based practices such as Motivational Interviewing, Harm Reduction Techniques, and Trauma-Informed Care principles.
  • Multilingual capabilities preferred but not required.
