Our services include clinical counseling, employee assistance counseling, organizational development consulting, staff augmentation, workshops, training, and supervision.

About the role

The Family Case Manager provides services in Montgomery County, MD. This is a full-time, exempt position with a schedule of Monday through Friday, 8am to 4pm, with some evenings required to meet with families or students.

Responsibilities

  • Client Referrals:
    • Accept referrals for social services.
    • Assist clients in assessing their situation, defining problems, and identifying acceptable courses of action through recommended community services.
    • Complete initial intakes and assessments.
    • Consult with county, community, and agencies to identify and develop resources for client assistance.
  • Family Service Plan and Individualized Client Services:
    • Analyze issues in each case to develop family service plans.
    • Determine intervention strategies and appropriate actions based on client needs.
    • Make follow-through contacts with clients and agencies to ensure services are provided.
    • Provide care management services for families with economic or social needs, including housing, healthcare, employment assistance, food, clothing, and education.
    • Focus on services for recently arrived migrant and asylum-seeking youth families as required.
    • Prepare and update case records and client status reports.
  • General Client Services:
    • Outreach and network with youth, families, communities, schools, and other agencies.
    • Provide ongoing social services, including crisis assistance, supportive counseling, food, clothing, housing, employment, transportation, and financial assistance.
    • Assist families with health insurance applications and provide information and referrals for community members and school staff.
    • Serve as an intermediary between clients and agencies, advocating for clients with public and private service providers.
    • Provide translation and interpretation services when available.
    • Co-lead support groups.
    • Collaborate with community school coordinators, therapists, school staff, and community partners to provide comprehensive care.
  • Case Management:
    • Ensure recordkeeping, including intakes, family needs assessments, family service plans, contact notes, and electronic database entry.
    • Stay updated on changes to rules, regulations, policies, and procedures related to program eligibility and service delivery.
    • Review programmatic and personal documents, create and maintain case notes, draft memos and letters, and perform related functions.
    • Conduct home visits as required by contract.
  • Training & Meetings:
    • Attend LTL trainings, events, and regular supervision.
    • Meet with Linkages team members to share information, training, and discuss case management issues.
    • Attend meetings with clients and schools when appropriate.
    • Attend Sheppard Pratt meetings and activities.

Requirements

  • Bachelor’s degree in a related field. Equivalent combination of education and work experience may be substituted.
  • Prior experience serving parents and families in need (internship experience considered).
  • Excellent collaboration skills.
  • Written and verbal fluency in both English and Spanish.
  • Experience working within collaborative school-based models of service delivery.
  • Prior experience or equivalent knowledge of immigrant populations and acculturation issues.
  • Thorough knowledge of community resources in the Montgomery County area.
