Position Summary
The Children & Family Health Institute (CFHI) is seeking an Early Intervention Service Coordinator to make a meaningful, lasting impact in the earliest years of life. The role involves conducting comprehensive assessments, connecting families with essential services, and staying current with best practices.
Key Responsibilities
- Build positive, collaborative relationships with families, caregivers, and team members while upholding the organization’s mission and values
- Conduct child and family interviews to assess developmental, social, and environmental needs
- Connect families with appropriate community resources and service providers
- Complete required reports and data submissions in alignment with grant and program guidelines
- Participate in child-find activities to identify service gaps and support early intervention efforts
- Stay current with best practices, trends, and innovations in case management and service coordination
- Follow all agency safety and infection-control procedures
- Maintain professional standards regarding attendance, appearance, and conduct
- Perform additional related duties as needed to support program success
Job Competencies & Minimum Qualifications
- Bachelor's degree in health-related discipline; RN or BSN; MSW preferred; or NJ certified teacher with Special Education certification
- 2+ years experience working with children (birth to 5 years) and families; prior home visit and case management experience preferred
- Strong computer skills
- Excellent verbal and written communication skills
- Strong organizational and interpersonal skills
- Valid NJ driver's license
- Bilingual in English and Spanish required
Working Conditions/Physical Demand
This job involves working outdoors and indoors with client/customer contact.
Pay
The compensation range for this position is USD $26.00 to USD $34.00.
