Bilingual Crisis Intake and Placement Case Manager

About the role
This position provides administrative support to the Mobile Crisis Response team at Pillars Community Health, collaborating with the Director of Crisis Services and Mobile Crisis Response Supervisors.
Responsibilities
- Develops and maintains resources for community linkages related to MCR services
- Maintains and updates the Mobile Crisis Response webpage
- Coordinates all Crisis program client assignments for post-crisis follow-up
- Affords assistance in facilitating the completion of consent forms with the client's family
- Attends hospital and community stakeholder meetings as needed
- Collaborates with hospital discharge planners to triage follow-up needs of clients
- Gathers and inputs data into electronic health records
- Maintains various logs and electronic client records to track and monitor key performance indicators and program outcomes
- Outreaches clients and client families and maintains all logs and tracking systems
- Follows up with clients and their family to identify and discuss resources, gaps in resources, and provide information about continued treatment options
- Collaborates with other intake staff throughout the organization to process client needs for services
- Supports continued post-crisis placement efforts in collaboration with other clinicians
- Participates in organizational meetings, in-services, monthly department meetings, and weekly staff meetings
- May maintain a small caseload to support case management needs of clients post-crisis services
Qualifications
- Bachelor's degree in counseling, psychology, social work, marriage and family therapy or related field required
- 1-2 years of direct client case management experience in social service, behavioral health, community health outreach, or related field required
- Prior experience coordinating health care for clients/patients is preferred
- Customer service experience required
- Bilingual (English/Spanish) required
- Positive, caring, and compassionate attitude toward mental health and those living with mental illness
- Excellent organizational and communication skills
- Initiative and self-starter attitude
- Ability to collaborate with other health care providers and work within different systems
- Effective communication with emergency room (ER) staff and other mental health providers
- Intermediate computer skills required
- Maintain a safe driving record and have a current valid Illinois Drivers License
- Personal vehicle needed for travel to/from office and community-based interventions
