About the role

Provides full performance level coordination of healthcare services for adults and children enrolled in the Resident's Access to a Coalition of Health (REACH) Program. Responsibilities include:

  • Eligibility determination for all Spanish-speaking applicants and enrollment of eligible applicants in the REACH Program using established criteria.
  • Outreach and care coordination to Spanish-speaking REACH members who are non-compliant to return them to care or assist them in accessing care.
  • Assistance to Spanish-speaking REACH members with accessing available resources within the REACH network, other state and county programs, and pharmacy assistance programs.
  • Teaching and assisting Spanish-speaking REACH members to understand their health status, develop self-care measures, and improve and maintain maximum healthcare status.
  • Outreach to community providers, groups, or organizations to educate them about services available through the REACH and Prescription Discount Programs.
  • Regular attendance at staff meetings, workshops, in-service programs, and educational offerings.

Requirements

  • High School Diploma or Equivalency (GED) or Bachelor’s degree in nursing, social work, psychology, education, counseling, or a related field from an accredited college or university.
  • Graduation from an accredited high school or possession of a high school equivalency certificate with two years of experience applying policies in a medical care, health insurance, or federal/state entitlement program and/or in coordination of care or possession of a bachelor’s degree in a related field.
  • Ability to communicate fluently, both verbally and in writing, in both English and Spanish.

Preferred Qualifications

  • Case management experience.
  • Ability to coordinate appointments and referrals with primary care and specialty care provider offices.
  • Data entry ability; proficiency in basic computer skills and systems.
  • Knowledgeable about health insurance.
  • Ability to apply and determine program eligibility criteria and follow standard operating procedures.
  • Enjoys working with diverse populations and attending community outreach events.
