Responsibilities
- Provide individual, family and group therapy sessions, as well as any other prescribed treatment service in accordance with agency and regulatory standards.
- Maintain required productivity levels and manage an assigned caseload effectively.
- Conduct initial intake and comprehensive assessments.
- Develop, implement and regularly monitor client treatment plans; collaborate with clinical team on treatment recommendations.
- Collaborate with external service providers to support client progress and continuity of care.
- Communicate regularly with courts, probation, parole, and other relevant external agencies to support client outcomes.
- Actively participate as a member of the interdisciplinary treatment team, including attending case conferences, team meetings, and in-service trainings.
- Complete and maintain accurate, up-to-date client case records in compliance with agency policies and other regulatory requirements.
- Prepare and submit all required reports, correspondence, and documentation in a timely manner, in line with agency, program, and regulatory deadlines.
- Maintain confidentiality in accordance with HIPAA, OASIS, and OMH regulations.
- Perform additional tasks, projects and responsibilities as assigned to support program operations.
Qualifications
- Master's degree in Social Work, Mental Health Counseling, or related field required.
- HPP credential required: LMSW, LCSW, LMFT, LMHC, LCAT; will consider candidates eligible for limited permit to be obtained within 90 days of hire.
- Minimum of 1 year experience providing individual and group counseling to individuals with substance abuse or mental health challenges (may include internship).
- Bilingual in Spanish and English speaking required.
