About the role
The Case Manager provides case planning, outreach, assessments, and crisis intervention to participants of The Center's LGBTQ-affirming wrap-around and support services. This role works collaboratively in a trauma-informed, goal-oriented team to help resolve housing stability for program participants.
Responsibilities
- Conduct intakes, assessments, and evaluations with new clients.
- Provide links to mental health services, substance abuse services, educational services, career development services, and other relevant resources.
- Deliver ongoing case management, crisis intervention, conflict resolution, and stabilization services to enrolled participants.
- Complete individualized service plans and self-sufficiency tools.
- Maintain accurate documentation of client goals, needs, case management plans, and service entries in internal data systems.
- Enter program data and outcomes into the region's Homeless Management Information System (HMIS) for grant and contract compliance.
- Document thorough case notes reflecting client progress toward individualized case management goals.
- Assist in completing monthly narrative reports for funder reports.
- Perform additional duties to support the organization's vision, mission, and values.
Requirements
- 1 year of experience providing case management services to people experiencing homelessness or at risk of homelessness.
- Fluency in English and Spanish.
- Comfort working in a PC/Windows-based environment with strong proficiency in Microsoft Office.
- Demonstrated ability to manage multiple tasks with shifting priorities.
- Competence in strength-based, trauma-informed, and Motivational Interviewing techniques.
- Reliable transportation, a valid driver's license, and auto insurance.
Preferred Qualifications
- Bachelor's degree in a social service-related field.
- 2 or more years of experience providing case management services to people experiencing homelessness or at risk of homelessness.
- Knowledgeable about current issues related to the LGBTQIA+ community and people experiencing homelessness.
Location
On-site at the office in Hillcrest, San Diego, and in the field with program participants.
Pay
$23.00–$25.00 per hour, commensurate with experience.
