About the Role
We are searching for a dedicated, compassionate, and highly organized individual to join our team as a Care Navigation Coordinator. This role is integral to fulfilling our mission of providing excellent, patient-centered care. The Care Navigation Coordinator will ensure seamless transition of care and excellent patient experience by coordinating patient onboarding, care coordination, referral coordination, and conducting health risk assessment (HRA) calls to develop personalized care plans.
Responsibilities
- Schedule and coordinate patients' first office visits to ensure a positive experience.
- Create patient charts and ensure all necessary documentation is complete.
- Verify patient eligibility and benefits.
- Request hospital records and other relevant medical information.
- Schedule first appointment with a care coordinator and primary care physician.
- Coordinate patients' referrals to specialists, hospitals, and other healthcare providers.
- Verify insurance coverage and obtain pre-authorizations as needed.
- Schedule appointments and communicate with patients, providers, and staff.
- Ensure accurate and timely documentation in our EHR system.
- Conduct health risk assessment (HRA) calls with new patients to establish risk levels and identify care needs.
- Collaborate with care coordinators to develop personalized care plans for new patients.
- Maintain accurate and up-to-date patient records.
- Communicate effectively with patients, families, providers, and staff.
- Participate in quality improvement initiatives and performance metrics.
- Adhere to regulatory requirements, policies, and procedures.
- Follow up on home health and DME orders to ensure patients receive services.
- Provide extraordinary customer service to all internal and external customers, including patients and team members.
- Utilize patient messaging tools.
- Work collaboratively with other care coordinators and the Marketing team to ensure patient satisfaction and good health outcomes.
- Additional duties as assigned.
Requirements
- 5 years of medical experience.
- Foreign MD license.
- Medical Assistance with more than 10 years of care coordination experience.
- LPN Florida License.
- Must be detail-oriented, possess the ability to multi-task and problem-solve, and be open to cross-functional training.
- Exercise proper phone etiquette and navigate computer software systems proficiently.
- Be team-oriented and work well with patients, colleagues, physicians, and other personnel in a professional and courteous manner.
- Have exceptional organizational skills with the ability to prioritize and complete tasks in a timely manner.
- Knowledge of medical terminology, CPT, HCPCS, and ICD coding preferred.
- Bilingual is a plus.

- Ability to use logical reasoning for simple and complex problem solving.
- Occasionally requires exposure to communicable diseases or bodily fluids.
- Occasional travel for clinic activities may be required.
