About the Role

To facilitate positive change in children with emotional and behavioral challenges along with their families, following the Wraparound process and the strength-based philosophy in partnership with stakeholders and service providers in the NJ System of Care network.

Responsibilities

  • Coordinate and facilitate the Wraparound process of working with children and families utilizing a strength-based team approach to ensure family’s needs are addressed and met.
  • Apply the wraparound model and process in carrying out care management duties.
  • Help families identify their strengths and needs and complete the Strength and Needs Assessments.
  • Use the strengths and needs of the child and family to guide development of Individual Service Planning (ISP).
  • Facilitate the Child Family Teams in developing the Individual Service Plans that identify strength-based strategies to address the child and family’s needs.
  • Collaborate and maintain regular contact with various system partners to monitor progress of and update the ISP (e.g., DCP&P, Probation, Juvenile Justice, school systems).
  • Update the Individual Service Plan (ISP) at least every 90 days based on ongoing assessment to determine if the interventions are implemented effectively and are generating positive outcomes.
  • Maintain ongoing engagement with the family and providers.
  • Connect families to available resources in the community, including housing, rental assistance, social service entitlements, domestic violence, outpatient mental health and substance abuse treatment facilities, food pantries, etc.
  • Advocate on behalf of families by attending meetings with families (school, DCP&P, court hearings, mental health appointments) to ensure the family’s voice is heard.
  • Serve on-call duty on a rotating basis to ensure 24/7 coverage by the agency.
  • Complete the following activities and deliverables per Medicaid requirements and Agency standards:
    • Weekly communication with families
    • Family visits at least once a month or more as needed
    • Bi-monthly communication with CFT members
    • Completion of 7-day Family Crisis Plans, 30-day Initial CFT, 90-day Comprehensive CFT
    • Documentation of progress notes within 72 hours of the activity

Requirements

  • BA/BS degree in a relevant field (e.g., social work, counseling, psychology, psychiatric nursing, criminal justice, special education) and one-year related experience or a Master’s Degree in a related field (e.g., social work, counseling, psychology, psychiatric nursing, criminal justice, special education).
  • MS degree desirable.
  • LSW licensure desirable.
  • Bilingual (Spanish, Portuguese, and/or Creole) a plus.
  • Knowledge of community resources.
  • Strength and Needs assessment certification.
  • NJ System of Care Association Care Manager/Wraparound Facilitator Training & Certification.
  • Effective communications (oral and written) in English and Spanish (if bilingual).

Pay

$50,000 - $54,000
