Job Summary
We are seeking a Bilingual Blended Case Management Supervisor to lead and support our case management team. In this role, you will supervise Targeted Case Managers, provide guidance on service delivery, ensure compliance with program standards, and oversee the coordination of care for adults in the community. You will play a key role in fostering a collaborative team environment and ensuring high-quality, client-centered services.
Key Responsibilities
- Assure that the DBH Practice Guidelines are put into operation to the extent possible within the general operations of the TCM duties.
- Provide daily on-site supervision to Teams of Blended Case Managers.
- Discuss the proposed daily contact for each staff member during Team meetings and address any problems they may need help with.
- Ensure that every effort is made to provide weekly individual supervision to each assigned staff member and document these sessions for review.
- Complete a chart review using the updated Utilization Review sheet during individual supervision, paying particular attention to whether or not a PGP exists to cover the time period since the last CBH review.
- Daily monitor the 30-day contact board and ensure that contacts are completed before an infraction occurs.
- Ensure that all Pay 4 Performance criteria are being met: staff under your supervision should complete documentation in a timely manner and ensure it meets general guidelines of legibility, is signed, dated, and follows the DAP format.
- Maintain productivity of the staff assigned to you using the reports provided through the TCM director.
Requirements
- Masters Level degree in social work, psychology, rehabilitation, activity therapies, counseling or education and Three years mental health direct care experience - one of which includes supervisory experience preferred,
- Knowledge of and the ability to put into practice the Blended Case Management Standards of Care,
- On-call responsibility & completion of BHTEN 10-week case management orientation, as well as a valid driver's license, is required,
- Bilingual proficiency in English and Spanish, with strong reading and writing skills is required,
- Current Driver's License; satisfactory driving record; eligible to operate program vehicles.
