BH Inpatient Discharge Planner - Adolescent Unit Therapy

About the Role

Coordinates and manages patient care and disposition planning for the Inpatient Psychiatric Unit.

Responsibilities

  • Coordinates inpatient and post-discharge care, scheduling, and reporting.
  • Provides disposition planning and maintains disposition data.
  • Coordinates client care with other service providers and collaborates with interdisciplinary team to ensure continuity of care.
  • Coordinates available NCH and community resources to enhance client care.
  • Provides education and support to patients and their families regarding treatment options and resources.
  • Completes documentation of discharge and care coordination activities in Epic and other related administrative paperwork in accordance with policy standards.

Requirements

  • Bachelor's Degree in social work, counseling, psychology, or a related field, required.
  • LSW licensure, required.
  • Experience providing and managing behavioral healthcare to children, adolescents, and families, required.

Skills

  • Excellent communication and interpersonal skills in order to establish and maintain positive relationships with clients, families, peers, and community representatives.
  • Ability to work with the hospital’s electronic medical records system.
  • Ability to work and problem-solve with minimal supervision.
  • Ability to adapt to change positively in workflow and process.
  • Spreadsheet, data entry and tracking, and computerized scheduling skills.

Physical Requirements

Occasionally: Blood and/or bodily fluids, cold temperatures, communicable diseases and/or pathogens, hot temperatures, lifting/carrying: 0-10 lbs, lifting/carrying: 11-20 lbs, loud noises, reaching above shoulder, working at heights, working outdoors.

Frequently: Bend/twist, climb stairs/ladder, color vision, depth perception, flexing/extending of neck, peripheral vision, seeing – far/near, sitting, squat/kneel, standing, walking.

Continuously: Audible speech, computer skills, decision making, hand use: grasping, gripping, turning, hearing acuity, interpreting data, problem solving, repetitive hand/arm use.

Additional: Demonstrate ability to pass CCM crisis management trainings once trained.
