BH Care & Administrative Support Coordinator
Job Summary
The primary role of the Behavioral Health Care Coordinator is to provide care management and administrative support to help patients access behavioral health, substance use, and social services tailored to their individual needs. This position supports the Behavioral Health (BH) team by facilitating service delivery, advocacy, and education for clients and the community. The Behavioral Health Coordinator also assists patients in connecting with additional resources, such as psychiatric services, referrals for a higher level of mental health care, and other community-based support services.

Essential Duties & Responsibilities
- Clerical and Administrative Tasks
- Assist BH providers with patient paperwork, faxes, and correspondence
- Ensure all patient communication is documented completely and scanned timely into the electronic health record (EHR)
- Follows the no-show policy and procedure and ensures all correspondences are documented in a telephone encounter or note in eClinicalWorks
- Coordinate phone triage for incoming calls to the Behavioral Health department
- Serve as a liaison for the medical and dental care teams
- Manage BH appointments with a focus on keeping the schedule full
- Support the BH provider in matching patients appropriately with an individual therapist
- Generate Behavioral Health (BH) reports as needed
- Verify patients’ insurance eligibility
- Under the guidance and supervision of the Behavioral Health Director, serve as a resource and train new and current non-behavioral HarborCHC staff on Behavioral Health protocols
- Maintain patient confidentiality and protect operations by keeping patient health information confidential as determined by HIPAA regulations
Qualifications
- Bachelor’s degree in social work or related field of study and 1 year of experience working in a behavioral health setting
- Ability to work with people of diverse cultural, educational, social, and economic backgrounds
- Ability to organize and prioritize work tasks
- Knowledge of HIPPA compliance for patient/agency records according to established professional social standards
- Ability to work as part of a multidisciplinary team
- Knowledge of community mental health and social services
- Computer and internet proficiency
- Coordinate long-term therapy referrals for patients with chronic/severe mental illness
- Excellent oral and written communication skills
- Excellent organizational skills
- Experience utilizing an Electronic Medical Records (EMR) system is highly desirable
- Bilingual English/Spanish required

Hours
HarborCHC is open Monday-Thursday 7:00am-7:00pm, Friday 8:00am-5:00pm, and Saturday 8:00am-5:00pm with occasional overtime or other shifts based on clinic need. This is a non-exempt position, and any overtime must be approved by the supervisor in advance.
