What Will You Do as the Care Team Coordinator?
- Administratively supports the Faith Hospice Bereavement Team
- Update information in MatrixCare, the electronic medical record
- Review client files to ensure they contain required documents; acquire any additional information required
- Cookmailings to clients and prospective clients
- Run required reports, stocks necessary supplies, supports Bereavement Team meetings

- Skilled with navigating various technologies and computer applications, including Excel
Education And Experience
- Knowledge normally acquired through completion of a health care administration degree or medical administrative assistant degree or similar
- Proficiency with computers and MS Office software
- Customer service experience in a health care setting

- Michigan driver’s license and vehicle liability insurance
Schedule
- Ideally, Monday - Thursday, 9:00am - 4:00pm
