Behavioral Home Health / Psychiatric RN (Full Time)

About the role

As a Registered Nurse – Behavioral Health, you play a key role in delivering compassionate, high-quality care to patients and families in their preferred home setting. We are seeking a Registered Nurse with strong clinical skills, commitment, and compassion.

Responsibilities

• Assess assigned caseload of behavioral health care patients and families to identify physical, psychosocial, and environmental needs as evidenced by documentation, clinical records, Interdisciplinary Team reports, after-hours reports, and on-site evaluations.
• Assume primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment.
• Assess patient needs and obtain data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care.
• Make the initial evaluation and re-evaluate the patient’s nursing needs during each visit; make follow-up visits according to patient, family, and facility needs.
• Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.
• Revise the plan of care in response to identified patient care issues and notify the Clinical Supervisor, physician, facility, and other team members.
• Counsel, instruct, and include the patient, facility, and family in following the Interdisciplinary Plan of Care.
• Teach behavioral health philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.).
• Initiate, supervise, and document observance of the behavioral health aide and LPN delivery of care per agency policy.
• Inform supervisor and/or facility supervisor of unusual or potentially problematic nursing facility issues.
• Provide appropriate support at time of death and perform bereavement assessment.

Requirements

• Current, unrestricted RN license.
• 1+ year clinical nursing experience.
• Able and willing to travel within branch/office coverage area.
• Valid driver’s license, reliable vehicle, and current auto insurance.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Associate’s or Bachelor’s in Nursing preferred.
• Home Health, Hospice, or HomeCare HomeBase experience preferred.
