Behavioral Health Therapist - Main IP - Full Time - Days

Responsibilities

• Direct Service Assessment: Provides service within parameters identified by Ohio Department of Mental Health and Addiction Services (ODMAS). Identifies psycho-social barriers to treatment. Identifies effects of illness on the patient and family/support systems. Identifies patient/family needs throughout the continuum of care, Identifies appropriate resources. Identifies age related changes. Develops a plan of intervention in collaboration with the interdisciplinary treatment team.
• Implement Plan of Care: Collaborates with the interdisciplinary team in developing treatment and discharge plans that reflect the patient’s level of functioning and identified needs. Provides individual and group interventions. Leads therapeutic activities to meet the needs of patients and families. Provides/assists with interventions related to crisis management, healthcare decision-making, discharge planning issues, elder abuse, bereavement, compliance issues, and other psychosocial barriers to recovery. Recommends strategies to ensure continuity of care and treatment in the least restrictive manner.
• Communicate Changes Promptly: Communicates changes in status and safety concerns promptly to the charge nurse and/or physician. Monitors and evaluates patient/family responses to the plan of care and interventions, modifying the approach as necessary.
• Documentation: Provides clear written communication related to the therapeutic plan and the individual's experience during therapeutic activities. Completes documentation in a timely and accurate manner.
• Professional Development: Maintains and seeks responsibility for professional development and licensure. Networks with colleagues to share ideas, trends, and innovations in areas of responsibility. Maintains membership and association in professional organizations, incorporating standards into operations to stay current with community and national practice trends. Shares knowledge and information with peers, other team members, and encourages changes in practice based on research and scientific principles.

Qualifications

• Education: Master’s degree in Counseling or Social Work required.
• Experience: 2 years Behavioral Health experience required. Experience with dual diagnosis preferred.
• Knowledge and Skills: Demonstrated knowledge of community mental health resources. Well-developed communication and interpersonal skills. Knowledge and experience with Cognitive Behavioral Therapy and/or Dialectical Behavioral Therapy preferred.
• Licenses and Certifications: Licensure as a Professional Counselor, Professional Clinical Counselor, Independent Social Worker, or Social Worker required.
