Behavioral Health Therapist (LCSW or LPC)

Qualifications

• MUST be an LCSW or LPC.
• At least 3 years of experience post licensure.
• Experience working with patients who have suffered from trauma, abuse and chemical dependency.
• Experience working with the healthcare needs of underserved populations.
• Experience in a health care setting helpful.
• Bilingual/Spanish speaking a plus.

Key Job Responsibilities

• Provides 1:1 psychosocial counseling for ELFHCC patients.
• Completes a psychosocial assessment that includes social, economic, cultural, age-related factors, history of child, elder or domestic abuse, prior psychiatric or substance abuse treatment, family and social support systems.
• Collaborates with other members of ELFHCC's behavioral health team to provide co-management activities for patients who require additional services.
• Develops and implements a comprehensive, interdisciplinary plan of care which includes patient preferences. Establishes a plan for regular contact with each patient to review progress and assess the potential for additional needs.
