Responsibilities
- Answers call bell- intercom system
- Assists with and de-escalates agitated or aggressive patients/family
- Performs hourly rounding
- Assists patients with Activities of Daily Living and Performs AM care/ PM care as instructed.
- Prepares patient and belongings for discharge
- Affords dietary necessities as follows: Set up meal trays, assists patient with feeding and/ or perform complete patient feed.
- Maintains Aspiration precautions
- Introduces self and role in care
- Observes or monitors behavior/health state and responses to therapy
- Maintains a safe environment and protects the patient from injury
- Maintains fall precautions, applies wrist restraints, applies vest restraints, provides continuous observation per policy
- Performs safe patient transfers utilizing proper ergonomics
- Applies and monitors patient restraints in a manner that protects the safety of patients and others per PAH policy
- Performs post mortem care
- Participates in unit programming and activities for patients and families
- Reinforces health care instruction to patients according to the directions of the nurse
- Documents findings, interventions and patients outcomes in the medical record under the supervision the nurse
Qualifications
- Bilingual in English/Spanish
- High School Diploma/GED
- Minimum of 2 years of experience (paid or unpaid) in a human service field involving direct contact with the individual receiving services (i.e. coaching, teaching, case management, etc.)
- Certified Nurse Aid (CNA) Certification by the State of Pennsylvania or RN without a BSN Degree
- Basic Cardiac Life Support (BCLS)
