Job Summary

This position is within Housing and Homeless Services with the Permanent Supported Housing Healthcare Collaborative (PSH HCC). This program is a collaboration between Integral Care, CommUnity Care, Central Health, and ECHO, designed to provide integrated, mobile healthcare to residents in designated Permanent Supported Housing single-site projects. This program works to fill service gaps for adult residents exiting chronic homelessness with disabling conditions. Individuals served present with high vulnerability and complex needs. Preferred for this position will be a working knowledge of best practices for Permanent Supported Housing. Interventions and services are typically provided in the community and require the use of a personal automobile and cell phone. Care Coordinators work in partnership with racially and culturally diverse individuals to define and facilitate their desired life outcomes. Care Coordinators participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments, and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services.

About the Role

Care Coordinators work in partnership with racially and culturally diverse individuals to define and facilitate their desired life outcomes. Care Coordinators participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments, and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services. Frequent service coordination with external and internal providers is necessary to support individuals in achieving independent functioning. Care Coordinators may provide all or some of the following: deliver therapeutic interventions fostering skills necessary to manage symptoms, obtain/maintain employment or housing, or to obtain services such as school, medical care, nutritional assistance, financial assistance, transportation, legal assistance, and resources fulfilling any basic need. Care Coordinators may also facilitate psychosocial rehabilitative skills training groups and/or utilize other evidenced-based interventions or therapeutic modalities. Although staff members in this position are not typically LPHA's, staff members in this position are working under the supervision of a LPHA who provides ongoing evaluation and oversight of all assessments, diagnoses, and treatment.

Responsibilities

Requirements

  • Current and valid driver's license and a clean driving record.

Qualifications

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field.
  • Master's degree preferred.

Pay

$24.18 - $25.39 per hour, based on education.

Schedule

Full-Time. Workdays: Monday - Friday. Workhours: 8:00AM - 5:00PM.
