About the role
The Behavioral Health Specialist contributes the psychosocial perspective to the interdisciplinary evaluation, assessment, plan of care, and disenrollment processes for Reid Health PACE Center participants.
Responsibilities
- Affirms psychosocial health status and social work needs using all available information resources.
- Collaborates with the interdisciplinary team to design psychosocial interventions aimed at keeping participants in the community and enhancing their quality of life.
- Participates in interdisciplinary evaluations, assessments, and plan of care for all participants.
- Serves as a participant advocate and liaison between the participant, family, and Care Team.
- Learns and leads within the Care Team to ensure continuity and coordination of care, and supports staff development.
- Engages in the Quality Management Program to support continuous performance improvement.
- Provides social work interventions such as individual participant contacts, collateral contacts, participant and family education, assessment, and counseling.
- Mobilizes resources and addresses mental health needs as they arise, ensuring participant needs are met.
- Ongoing case management and advocacy to ensure participant needs are addressed.
- Performs medical cart documentation in accordance with established standards.
- Complies with CMS PACE regulations pertinent to this role.
- Continuously develops professionally through participation in staff meetings and educational opportunities to maintain competence and licensure.
Requirements
Education Required: MSW
Education Preferred: LCSW
Experience Required: 1 year experience in a hospital, nursing home, or community-based setting
Experience Preferred: 2+ years post-Masters experience; Mental Health knowledge, Geriatric experience, Community Resources, Medical terminology, Crisis Intervention, Ongoing Case Management
