Behavioral Health LPN Per Diem

Responsibilities

1. Affirms patients within scope of practice at every home visit.
2. Carries out treatments per MD orders and reports back to RN case manager and/or clinical manager.
3. Reports all changes in condition to MD.
4. Provides patient/caregiver education as per care plan.
5. Completes all documentation within 24 hours, including changes in condition and follow-up with provider or other disciplines with supporting documentation.
6. Attends all staff meetings, inservices, and other required meetings.
7. Complies with all mandatory requirements, including Healthstreams.
8. Fulfills all policies and procedures.

Qualifications

• Graduated from an approved Practical Nurses Education Program.
• Minimum one year experience in the acute care setting. Home care experience preferred but not required.
• Licensed Practical Nurse with valid (or eligible) Connecticut license required.
• Valid driver's license and proof of car insurance required when using a personal vehicle for company business.
• Access to a dependable vehicle for daily travel to multiple stops.
• CPR certification required and must be maintained within 3 months (90 days) of hire.
