About the role

Oversee the medical needs of youth within Foster Care, including conducting screenings, treatment planning, case management, care coordination, health promotion, and youth and family support and services while using technology to link services.

Responsibilities

• Conduct screenings, including HIV risk screenings, intake screenings, and obtain family health history.
• Perform routine screening for substance abuse, developmental health, mental health, and coordinate physical body checks for abuse.
• Provide routine management and training regarding health education.
• Establish treatment goals, utilizing historical and current available information in collaboration with a multidisciplinary team.
• Reinforce instruction of medication for youth and both birth and foster families on medication administration.
• Assist Psychiatric Mental Health Nurse Practitioner with:
  • Completing Prior Authorizations
  • Calling pharmacies for medication verification
  • Calling/faxing labs to schedule lab draws and get results
  • Calling providers for follow-up and records release
• Educate families and youth on safe medication storage and possible side effects to watch for.
• Perform medication reconciliation and monitoring of medication.
• Support youth and families on how to maintain medical supplies and equipment.
• Follow up after medical/dental appointments, community provider visits, urgent/emergency care, or hospitalization.
• Provide medical case management and documentation for medical, dental, and behavioral health appointments and updates.
• Maintain compliance with state and agency law and regulations.
• Adhere to agency policies and procedures; support agency mission, vision, and values, including participating in internal surveys and data collection for continuous improvement initiatives.
• Practice proper infection control.
• Perform other duties as assigned.

Requirements

• Current New York State Licensed Practical Nurse license and CPR/First Aid certification.
• Minimum 1–3 years of experience. Experience with at-risk youth preferred.
• Flexible schedule and ability to provide weekend, holiday, and On-Call coverage.
• Must have a current, valid driver’s license per Agency guidelines. Travel required.
• Ability to work with a culturally and economically diverse population.

Pay

$59,500 per year
