Behavioral Health Crisis Worker

About the role

This position involves planning and implementing social work services for patients and families, conducting crisis assessments, and arranging appropriate dispositions. It also includes discharge planning, guardianship petitions, and end-of-life decision-making.

Responsibilities

• Performs intake assessments and evaluations of clients, including personal, social, and emotional development.
• Participates in multidisciplinary treatment planning, addressing special treatment needs based on age, gender, sexual orientation, or ethnic origin.
• Assesses patients for discharge planning needs and petitions for guardianship/conservatorship.
• Interprets social, psychological, and emotional problems and shares assessments with the team.
• Evaluates patients for signs or symptoms of abuse and coordinates referrals and counseling.
• Assists patients and families with end-of-life decision-making, advance directives, and discussions on patient rights.
• Creates patient Behavioral Health Treatment Plans based on admitted psychiatric problems/diagnoses.

Requirements

• Minimum Qualifications: Bachelor’s degree in social work, psychology, sociology, or related field; BLS certification within 30 days of hire; ACADEMIC TRAINING: AVADE training within 30 days of hire.
• Preferred Qualifications: Master’s degree; 2+ years experience in a behavioral health setting.
