About the role
We are seeking a compassionate, licensed Behavioral Health Counselor to join our integrated care team. The role involves providing behavioral health assessments, therapeutic interventions, treatment planning, patient education, and care coordination in a Federally Qualified Health Center environment.
Responsibilities
- Conduct comprehensive behavioral health assessments, screenings, and biopsychosocial evaluations using validated tools.
- Develop individualized treatment plans with measurable goals and evidence-based interventions.
- Provide individual, group, and family therapeutic interventions for mental health and substance use concerns.
- Perform crisis intervention, safety assessments, and de-escalation support as needed.
- Career coordination with medical providers, nurses, case managers, social workers, and community partners.
- Complete referrals and support patients in accessing community-based services and resources.
- Maintain timely, accurate, and confidential documentation in the electronic health record.
- Participate in case conferences, clinical supervision, staff meetings, and required trainings.
- Promote trauma-informed, culturally responsive, patient-centered care across all interactions.
Requirements
- Active Louisiana licensure as an LCSW or LPC is required.
- A Master’s degree in Social Work or Counseling is required.
- Experience conducting assessments, formulating diagnoses, and developing treatment plans.
- Knowledge of evidence-based practices, including trauma-informed care and treatment for co-occurring disorders.
- Working knowledge of DSM and ICD diagnostic frameworks.
- Strong interpersonal, communication, and crisis intervention skills.
- Proficiency with electronic health records and timely clinical documentation.
- Knowledge of HIPAA, confidentiality requirements, and integrated care best practices.
- American Heart Association BLS for Healthcare Providers required.
- Valid driver’s license and reliable transportation are preferred if travel between sites is required.
Qualifications
- One year of post-graduate paid clinical experience in mental health and/or substance use disorder treatment is preferred.
- Experience in an FQHC, community health, integrated care, or multidisciplinary setting is also preferred.
