About the role
The Behavioral Health Consultant works as part of the primary care team to manage patients with behavioral health problems efficiently and effectively. They provide timely interventions, transition patients with chronic behavioral conditions to appropriate services, link families to resources, and provide referrals. They also educate patients and staff regarding behavioral health care, prevention, and treatment.
Responsibilities
- Provides timely interventions
- Transitions patients with chronic behavioral conditions to appropriate services
- Links families to resources and provide referrals
- Edits patients and staff regarding behavioral health care, prevention, and treatment
- Documents interventions in the electronic medical record, in accordance with compliance requirements and guidelines
- Participates in evaluation and training of peers and support staff
- Makes recommendations for improvement based on monitoring of behavioral health program
- Promotes a positive work culture and upholds clinic policies and procedures, including the WE CARE standards of Behavior
Requirements
- Professional and Technical Knowledge: Understanding in the application of behavioral health theory, case management interventions, and ethical practices of the behavioral health profession. Requires a Master’s Degree in Social Work, Counseling, or a related field.
- Licenses and Certifications: License to provide behavioral health interventions/counseling (LMHC, LMHC-A, LICSW).
- Technical Skills: Ability to prepare basic correspondence and simple reports in Microsoft Word, create tables and simple displays of information in Microsoft Excel, create basic presentations in Microsoft PowerPoint, and enter and manage data into a centralized electronic health records.
Qualifications
- Bilingual/Biliterate in English/Spanish
- Valid Driver’s License

$78,441.60 - $97,522.00 DOE with ability to go higher for highly experienced candidates.
