Essential Functions
- Provides comprehensive assessment and diagnosis of behavioral health clients
- Provides effective treatment planning and assists clients in successfully achieving goals
- Provides assessment, consultation, and brief intervention for psychological/psychiatric problems and/or disorders
- Aids in the development of care management processes such as the use of guidelines, disease management techniques, case management, and patient education
- Evaluates crisis situations and applies appropriate interventions
- Aids in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration
- Works as a member of the Care Team to improve health outcomes
Job Requirements and Qualifications
- Education: Bachelor’s Degree in Social Work or related field; or Master’s Degree and licensed in counseling, social work, or a related field
- Experience: A minimum of two (2) years of counseling experience required; Experience in delivering behavioral health services required; Knowledge of DSM-V required; Experience in an integrated health care environment preferred; Experience in Medicaid, Medicare Advantage, and NCQA Standards preferred
Additional Information:
- Clinic hours are Monday - Friday 8 a.m. to 5 p.m.
