Behavioral Health Community Housing Specialist

About the Role

Facilitates recovery and enhances wellness by providing housing support and care management to increase individuals' ability to live independently in the community. Assists individuals in obtaining and retaining living arrangements of their own choice, becoming socially and physically integrated into the community, and reducing the need for more intensive behavioral health services. Develops and maintains positive working relationships with community agencies and participates in multidisciplinary treatment team meetings to collaboratively support individuals in housing support programs.

Responsibilities

• Actively facilitates treatment planning, utilization management, and discharge planning with the individual receiving housing services and the treatment team.
• Coaches and/or teaches skills related to psychoeducation, understanding diagnosis, triggers, basic coping skills, advocacy, understanding stigma, empowerment, and recovery support.
• Coaches and teaches skills related to Activities of Daily Living, including but not limited to food preparation, meal planning, laundry, personal hygiene, household organization, employment/volunteer activities, organizing personal schedule, budgeting, transportation, socialization, and/or other housing goals.
• Provides assistance with the location of housing, roommate matching, renter skills training, and/or landlord/tenant negotiations as outlined by the housing program.
• Completes all required documentation for individual services (e.g., Intake Assessment, Outcome Measures, C-SSRS, Treatment Plan, Safety Plan, and other clinical documentation).
• Acquires knowledge of resources for Lebanon County while maintaining an excellent working relationship with all providers and community agencies.
• Acts as a liaison with other community agencies/groups, funding sources, and businesses related to services provided to individuals in housing programming.
• Provides support, assistance, and consultation to families, social supports, landlords, and community agencies of the individual.
• Assists in providing access to behavioral health services throughout Lebanon County.
• Supports individuals in community settings, such as teaching community safety, assisting with transportation efforts (e.g., public transportation), supporting identified spiritual needs, and accompanying individuals to socialization activities related to their goals.
• Serves as an ongoing liaison to community resources through referrals and accompanying individuals to appointments when appropriate.
• Completes necessary documentation for each individual encounter within 48 hours of provided service.
• Attends all meetings with supervisors, staff, and team meetings, as well as trainings and in-services.
• Models respect, trauma-informed care, and the recovery model in all interactions.
• Advocates and informs regarding mental health and resiliency.

Requirements

• High School Diploma or GED required.
• 2 years of related work experience required.
• Must possess a valid driver's license in the current state of residence. Driving record must meet requirements established by WellSpan Risk Management.
• Be trauma-informed, culturally competent, and recovery-oriented in practice.

About The Team

WellSpan Philhaven, situated on more than 200 acres in Mt. Gretna, PA, is a tranquil campus with walking trails nestled within a picturesque forest and adjacent to an operating farm. The campus provides an atmosphere of peace and serenity for our clients. Our most comprehensive continuum of care, including more than 20 programs and services, is located at this main campus. At WellSpan Philhaven, we help address mental health conditions such as depression, anxiety, bipolar disorder, attention deficit disorder, phobias, post-traumatic stress disorder, and more.

Schedule: Full-time, Days/Evenings, 80 hours per 2 weeks.
