At Reema Health, we're dedicated to providing personalized, community-based care for individuals living with serious mental illness and substance use disorder. We seek an experienced Community Guide who can build meaningful relationships and support members in achieving their health and wellbeing goals.
Key Responsibilities
- Engage and work alongside diverse clients, focusing on those with severe and persistent mental illness, substance use disorders, and complex medical conditions.
- Reach out to members to enroll them in our care model and provide high-quality support to help them achieve improved health and wellbeing outcomes.
- Assess member needs, support systems, and behavior to guide them through enrollment in behavioral and mental health services.
- Collaborate with clinical therapists and prescribers to provide holistic support for members and help them connect to care.
- Coordinate referrals and support appointment scheduling and attendance with various providers for members.
- Utilize text, phone, email, and face-to-face interactions to match communication approaches with each member's preferences and situations.
- Identify and connect members to community and health resources to support their needs, including transporting members when necessary.
- Prepare members for services ending by fostering self-sufficiency and independence.
- Document and record interactions accurately and promptly in the Reema App, ensuring confidentiality and compliance with HIPAA.
- Work independently and collaboratively in a fast-paced startup environment, adapting to frequent changes and opportunities.
Qualifications
- Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences, or related field
- 3+ years of direct experience with the population or community to be served
- Experience with crisis planning, case management, boundaries, and personal safety in community work
- Proficiencies in critical thinking, time management, strategizing, and multitasking
- Strong relationship-building and communication skills
- Proficiency with mobile devices and web-based platforms
- Ability to troubleshoot technical issues and work independently
Requirements
- 18 years of age or older
- Flexible and reliable access to transportation, valid Driver's License, Vehicle Insurance, and a satisfactory driving record
- Pass a pre-employment drug screening and background check
- Comply with federal, state, and local regulations regarding patient confidentiality (HIPAA)
- Use technology-based tools and systems daily
- Lift and carry up to 25 pounds occasionally

Pay Range
$50,000 - $55,000 USD per year (Kansas City, KS)
