Role Overview: A behavioral health clinician collaborates with primary care teams to triage, manage, and provide mental health services to patients within primary care settings. They conduct diagnostic assessments, deliver brief behavioral interventions, and refer patients to specialized services as needed. The role may include managing a small outpatient caseload for ongoing therapy. Qualifications: Candidates must hold a Master’s Degree in Social Work and be licensed as a Licensed Clinical Social Worker (LCSW) by the Illinois Department of Financial and Professional Regulation. A minimum of 2 years of experience as a therapist with competency in cognitive and behavioral interventions for individuals, families, couples, and groups is required. Strong interpersonal skills and collaboration with healthcare teams are essential. Key Responsibilities Provide consultation and coordinate care with primary care staff.Identify, refer, and advocate for patients needing specialized behavioral health services.Assist in detecting “at risk” patients and develop plans to prevent deterioration.Deliver solution-focused behavioral interventions and educate patients and families on care and prevention techniques.Support primary care providers in recognizing and treating mental health and psychosocial challenges. Additional Duties: Participate in meetings and quality improvement activities to enhance patient care. The position offers a sign-on bonus and a competitive hourly rate.
