Behavioral Health Case Manager (Re-entry) - Paducah, KY
Primary Responsibilities
- Provide a complete continuum of quality care through close communication with members via phone, virtual, or in-person interaction
- Absorb members with the transition from a carceral facility back to the community via Health Assessment and Care Planning
- Partner with care team (correctional facilities, community providers, internal staff) to complete care plans and warm handoff
- Improve health and self-management of conditions
- Reduce inappropriate emergency department utilization, preventable hospital readmissions, and reduction in recidivism
- Provide member education on Medicaid benefits, treatment options, and comprehensive services
- May participate in community events related to scope of work
- Maintain compliance with NCQA Population Health Management Standards
- Maintain compliance with state-specific contractual requirements related to the delivery of care management
Required Qualifications
- You must have 1 of the following licenses:
- Independently Licensed Mental Health Clinician with a Master's degree in psychology, social work, counseling or marriage, or family counseling, or related mental health discipline
- A registered nurse (RN) who is certified by the American Nurses Credentialing Center (a subsidiary of the American Nurses Association) as a psychiatric nurse or mental health clinical nurse specialist
- A registered nurse (RN) who has a master's degree in nursing with a specialization in psychiatric/mental health and 2 years of supervised clinical experience and is licensed to practice as a psychiatric or mental health nurse if required by the state of practice.
- Licenses must be independent, active, and unrestricted in the state of Kentucky
- 2+ years of post-licensure experience in a related mental health environment
- 1+ years of direct experience with justice-involved populations
- Proficient computer skills and good working knowledge of Microsoft Word and Outlook
- Access to high-speed internet and a dedicated, distraction-free workspace at home
- Driver’s license, access to reliable transportation and the ability to travel within designated service delivery area
- Permanent residence in the state of Kentucky
Preferred Qualifications
- Case Management experience
- Carceral facility experience
- Community Mental Health experience
- Experience working with the Medicaid population
- Experience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients
- Experience working with low-income populations
- Experience working with either Medicaid or Medicare funded programs
- Medical / Behavioral setting experience (i.e., hospital, managed care organization, or joint medical / behavioral outpatient practice)
- Dual diagnosis experience with mental health and substance abuse
Pay and Benefits
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
The salary for this role will range from $60,200 to $107,400 annually based on full-time employment.
