Behavioral Health Case Manager I - New York Medicaid

About the role

This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands-on engagement.

Responsibilities

• Uses appropriate screening criteria knowledge and clinical judgment to assess member needs.
• Conducts assessments to identify individual needs and develops care plan to address objectives and goals as identified during assessment.
• Maintains and evaluates the effectiveness of the care plan and modifies it as needed.
• Supports member access to appropriate quality and cost-effective care.
• Collaborates with internal and external resources to meet identified needs of the members.

Requirements

• Requires MA/MS in social work, counseling, or a related behavioral health field or a degree in nursing, and minimum of 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment.
• A current active unrestricted license such as LCSW (as applicable by state law and scope of practice), LMHC, LMSW (as allowed by applicable state laws) in the state of New York is required.

Preferred Skills, Capabilities and Experiences

• Experience in case management and telephonic and/or in person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders preferred.

The Behavioral Health Case Manager I will be compensated based on a number of legitimate, non-discriminatory factors set by the Company. Monday - Friday, 8:30 AM - 5 PM EST. No weekends or holidays.
