Behavioral Health Case Manager
Oversee primary needs of patient
Oversee primary needs of patient, including assessment of client and family/caregivers, and develops comprehensive plan of care.
Provide primary home care/consultation to complex client and family needs within area of expertise and in collaboration with the physician.
Monitor patient care through assessments, evaluations and/or patient records.
Schedule
Schedule:8:00 a.m.- 4:30 p.m, every Friday, every other Saturday, rotating Sundays and weekdays.
Minimum Requirements
- Licensure / Certification / Registration: Counselor credentialed from the Illinois Department of Financial and Professional Regulation preferred.
- Social Worker credentialed from the Illinois Department of Financial and Professional Regulation preferred.
- Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
- Education: High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
Additional Preferences
- Master's Degree required
- Behavioral health inpatient experience HIGHLY preferred
