RHD Glassport is a program for people with behavioral health issues who are transitioning into the community. The program provides supportive services to enhance residents' lives while maintaining stable mental health, with the goal of facilitating independence and improving quality of life.

Responsibilities

• Perform the delivery of counseling and clinical support services:
  • Partner with individuals to develop their recovery goals in relation to their medical, psychiatric, and/or substance abuse disorders.
  • Conduct individual and group counseling sessions.
  • Assist in program participants’ aftercare planning and follow-up.
• Caseload Management:
  • Evaluate residents’ progress and report to funders/referral sources as requested through authorization/re-authorization reviews.
  • Participate actively in clinical case conferences as a member of the individual’s support team.
  • Maintain active caseload including engaging individuals in ongoing treatment.
  • Maintain tracking system for treatment plans and other required documents to meet program funder guidelines and program goals.
  • Participate in clinical group meetings regarding ongoing treatment goals, progress, and discharge as needed.
• Administrative:
  • Enter documentation into electronic medical records in accordance with agency guidelines.
  • Maintain up-to-date scheduling calendar.
  • Perform other duties as assigned.

Requirements

• Bachelor’s degree in Social Work, Psychology, or related field.
• Certification may be required to be obtained within a specific timeframe of the hire date, depending on program license requirements.
• A minimum of two years of experience working with individuals with co-occurring disorders.
• Working knowledge of computer business software and electronic health care record programs.
