Job Duties

  • Interviews clients and family members to obtain psychosocial histories relevant to medical, psychiatric, and behavioral health concerns.
  • Conducts comprehensive assessments during initial client encounters and evaluates ongoing treatment needs.
  • Develops treatment plans and coordinates appropriate referrals to internal and external providers, agencies, and community resources.
  • Provides evidence-based psychotherapy interventions, including cognitive behavioral therapy and other clinically appropriate treatment modalities.
  • Documents assessments, treatment plans, interventions, and patient progress in the electronic medical record in accordance with departmental and regulatory requirements.
  • Responds to patient inquiries, triages clinical concerns, and assesses the need for behavioral health services and interventions.
  • Serves as a liaison between patients, families, physicians, nursing staff, trainees, and community agencies to facilitate coordinated care and continuity of services.
  • Assists with patient access initiatives, referral management, waitlist oversight, scheduling coordination, and resource navigation to support efficient clinic operations.
  • Supports group therapy programs and other behavioral health services through coordination, implementation, and patient engagement activities.
  • Participates in quality improvement initiatives, program development efforts, and operational projects designed to enhance patient care, clinic efficiency, and service delivery.
  • Participates in research activities as appropriate, either directly or through facilitation of study recruitment, patient engagement, and program support.
  • Provides clinical education and training to learners and staff as appropriate.
  • Provides supervision and mentorship to social work interns, fellows, and trainees in accordance with applicable licensure requirements.
  • Assists with continuity of care planning, including coordination of services, follow-up care, and transportation arrangements when appropriate.
  • Collaborates with multidisciplinary teams to support patient care, clinic operations, and departmental initiatives.

Minimum Qualifications

  • Master’s degree in a relevant field.
  • Two years of relevant experience.
  • Licensed Marriage & Family Therapist (LMFT), Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), Licensed Clinical Social Worker-Supervisor (LCSW-S), and Licensed Professional Counselor-Supervisor (LPC-S).
