Responsibilities
- Supports ECM students with behavioral health conditions, focusing on those with SMI and/or SUD needs.
- Works primarily with a caseload of students with behavioral health needs (majority Tier 1 or 2).
- Utilizes brief behavioral health therapeutic interventions to help students manage their health.
- Provides brief crisis interventions and makes warm hand-offs to local crisis resources as needed.
- Provides education to students and families about chronic medical and behavioral health conditions to improve health literacy.
- Trains and supports ECM students on behavioral health conditions, including treatments and evidence-based treatments.
- Affords assistance in coordinating medical and behavioral health access issues with PCP offices, specialists, and ancillary services.
- Tracks medical and behavioral health outcome measures in a patient registry.
- Ensures smooth transitions of care, collaborating with psychiatric hospitals for M1 and with the IEHP transitions of care team for M2, regarding client admissions/discharges.
- Collaborates on student care issues with other ECM Care Team members, participating in weekly systematic case reviews and ad hoc case reviews.
- Aids RN Care Managers in reviewing students’ Comprehensive Health Needs Assessments.
- Engages with students, both in-person and on the phone, using evidence-based approaches (like Motivational Interviewing) to promote collaboration and increase student control over their health.
- Provides consultation for all students of the team when discussing clinical tasks.
- Supports the learning and practice of the Teaching and Therapeutic Community model and trains faculty that incorporates essential elements of this model.
- Attends workshops, meetings, and trainings as requested by supervisor.
Requirements
- Master’s degree in behavioral health, public health, or a related field.
- Licensed or registered professional in good standing with the California Board of Behavioral Sciences (BBS) as ASW, LCSW, LMFT, AMFT, LPCC, or APCC.
- Minimum of 5 years clinical experience, with at least 2 years in care management, population health, or related fields.
- Expert knowledge of clinical practices, care coordination, and patient-centered care.
- Strong analytical skills with the ability to interpret clinical data and translate findings into actionable strategies.
- Previous clinical experience working with individuals recently released from jail or prison is preferred.
- Previous supervisory experience is preferred.

- Proficient in Microsoft Office Suite or similar software.
- Knowledge of contracting process and associated local, state, federal, and other regulations.
- Ability to multi-task, identify problems, provide recommendations to management teams, and implement applicable systems.
- Ability to work constructively with diverse people and with parties that may have divergent perspectives and interests.
- Understanding and appropriate application of Amity Foundation policies and procedures.
