Behavior Health Management Care Coordinator

Location: Woodbridge, VA

Shift: First (Days) – standard working hours 8:00 AM to 5:00 PM EST, Monday through Friday

About the role

This position works with members under Sentara Health Plans who are both medically and behaviorally complex. The role involves conducting in-person/face-to-face assessments as well as remote telephonic assessments for members with Serious Mental Illnesses, Serious Emotional Disturbances (ages 2+), and Substance Abuse Disorders.

Applicants must reside in Northern Virginia (NOVA), Culpeper, Louisa, or Fredericksburg. Travel may include up to 90 minutes one way.

Requirements

  • Bachelor’s degree (required)
  • Valid state license in the state of practice. One of the following:
    • Licensed Clinical Social Worker (LCSW) – Virginia Department of Health Professionals (VADHP) or North Carolina Social Work Certification and Licensure Board (NCSWCLB)
    • Licensed Professional Counselor (LPC) – Virginia Department of Health Professionals (VADHP) or North Carolina Board of Licensed Clinical Mental Health Counselors (NCBLCMHC)
    • Licensed Marriage and Family Therapist (LMFT) – Virginia Department of Health Professionals (VADHP) or North Carolina Marriage and Family Therapy Licensure Board (NCBMFT)
    • Licensed Substance Abuse Treatment Practitioner (LSTAP) – Virginia Department of Health Professionals (VADHP)
    • Registered Nurse (RN) – Single State or Multi-State license from Virginia Department of Health Professionals (VADHP) or North Carolina Board of Nursing (NCBON)
  • 2 years of behavioral health experience (required)
  • 1 year of case management experience (required)
  • 1 year of experience with Children/Adolescents (required)
