About the role

Join our multidisciplinary Community Mental Health and Wellness team at Wilder in providing clinical service delivery that supports our community’s mental wellness, critical to Wilder’s mission of improving lives today and for generations to come. As part of this innovative Certified Community Behavioral Health Clinic (CCBHC) model, you will collaborate closely with other service partners to ensure coordinated, integrated care for clients and their families.

Wilder is known nationally for our culturally-specific mental health services. You will join a team that brings diverse identities, cultures, lived experiences, and skills to provide trauma-informed, culturally responsive, and impactful mental health and wellness services for the whole family.

Responsibilities

• Provide Adult Targeted Case Management (TCM) to assigned clients who meet program eligibility, including assessment, treatment planning, service engagement, crisis intervention, documentation, and case coordination.
• Coordinate integrated care by working with other healthcare professionals.
• Monitor services, provide information and referral, and conduct home visits.
• Complete required documentation such as client progress and services delivered.
• Provide work direction, program, and/or project leadership as needed.
• Work within a culturally specific program; must be bilingual in Hmong/English, Karen/English, Khmer/English, or Vietnamese/English.
• Explain documents/information to clients with marginal English language skills.

Requirements

• Bachelor’s Degree in Social Work, Psychology, Counseling, or related Behavioral Health Degree required.
• At least 2,000 hours of supervised experience in the delivery of mental health services, including 38 hours per year of regular ongoing supervision (at least one hour per month must be clinical supervision regarding individual service delivery with a case management supervisor).
• If you do not have 2,000 hours of supervised experience:
  • Begin 40 hours of commissioner-approved training in case management skills and mental illness characteristics before starting case management services.
  • Receive weekly clinical supervision from a mental health professional until 2,000 hours are met.
• Without a bachelor’s degree, you must meet one of the following:
  • Three or four years of experience as a case manager associate.
  • Be a registered nurse without a bachelor’s degree with three years of specialized training in psychiatry and community interaction or discharge planning in a mental health setting.
  • Qualified as a case manager under the 1998 Department of Human Services waiver provision with continuing education, supervision, and mentoring.
• Immigrants without specified qualifications may provide case management to adult immigrants with serious and persistent mental illness if they:
  • Are enrolled in and actively pursuing a bachelor’s degree in behavioral sciences or a related field.
  • Complete 40 hours of required training.
  • Receive weekly clinical supervision until requirements are met.
• Foundation Driver required: Must drive Foundation vehicles, clients, or both (valid driver’s license implied).

Pay

$52,000 - $57,000 per year
