Responsibilities

• Deliver quality clinical care and support to patients in their homes
• Utilize electronic documentation via tablets in patient homes
• Collaborate with team members to achieve patient-centered outcomes
• Participate in paid training, mentorship, and career development opportunities
• Maintain compliance with licensing, CPR, TB testing, and other health requirements

Qualifications & Skills

• Active, unencumbered LPN/LVN license in the practicing state
• Current CPR certification with hands-on component
• TB skin test within the last 12 months
• Reliable transportation
• Six months of nursing experience preferred but not required
