About the Role

JAG Physical Therapy is seeking a skilled Physical Therapist specializing in orthopedic rehabilitation in the home setting for patients recovering from total hip and knee replacement surgeries and other orthopedic-related conditions. Our mission is to help patients regain mobility, strength, function, and confidence in the comfort of their own homes. This approach emphasizes not only physical recovery but also the emotional and social benefits of re-engaging with the community.

Physical therapists will evaluate and treat patients for the duration of their plan of care in their home setting, with the goal of transitioning them into one of our JAG outpatient orthopedic locations for continuity of care.

Responsibilities

• Educate patients on safe bed mobility, transfers, and ambulation in consideration of post-operative precautions.
• Transfer, ambulate, and perform stair negotiation with patients to determine safety for discharge to home.
• Collect and record patient care data following the format and guidelines of patient care services, regulatory agencies, and third-party payers, including initial evaluations, daily notes, reassessments, progress reports, discharge plans, and communication with team members.
• Complete daily billing and follow department guidelines for productivity.
• Prepare for evaluations by obtaining patient history, researching unfamiliar diagnoses, and exhibiting knowledge of patient diagnoses.
• Select and administer tests and measures, synthesizing evaluation results to formulate diagnoses, goals, prognosis, and plan of care.
• Establish treatment plans based on evaluation results, goals, and best practices, collaborating with the interdisciplinary team, patients, and their support systems.
• Assess and re-assess the need for modifications to the plan of care and goals.
• Provide education and training to patients and their support systems.
• Perform functional training, including ADLs/IADLs, therapeutic activities (e.g., transfers, bed mobility), rehabilitation equipment prescription and training, and community reintegration.
• Responsible for treatment, plan of care, and exercise prescription for patient caseload.
• Provide safe, effective, and compliant interventions in accordance with JAG standards.
• Document all treatments using Prompt EMR.
• Attend the annual JAG Billing and Coding seminar.
• Communicate with Manager/Patient Access team for scheduling evaluations once demographics are received.

Requirements

• Bachelor’s or Master’s Degree required; Doctorate degree preferred.
• Licensed Physical Therapist in the state of practice (NY, NJ).
• Completed state-regulated CEU requirements.
• Valid CPR license.
• Experience/knowledge with EMR software is preferred.
• Excellent oral and written communication skills.
• Strong clinical decision-making skills.
• Excellent work ethic and dedication to patient success.
• Basic computer skills.
• Ability to work as a team member and individually with minimal supervision.
• Maintains a positive, professional demeanor at all times.
