About the role

The Associate Healthcare Director (AHCD) assists the community's Healthcare Director (HCD) in leading health and wellness activities for the resident population and ensures efficient and effective operations of the resident care department.

Responsibilities

  • Evaluates residents prior to move-in to determine care and service needs.
  • Conducts re-evaluations as needed based on resident conditions and state regulations.
  • Develops and revises individualized service plans based on evaluations and resident preferences.
  • Collaborates with physicians, pharmacists, and other healthcare professionals to coordinate care.
  • Maintains the medication management program, conducts training, audits, and other safety activities.
  • Maintains knowledge of relevant regulations and ensures compliance.
  • Collaborates with the HCD to ensure the resident care department is survey-ready.
  • Participates in developing and implementing plans of correction.
  • Maintains infection control standards to prevent or limit the spread of communicable illnesses.
  • Trains and validates the skills of medication technicians, personal care aides, and others.
  • Conducts interviews and provides onboarding training/coaching for new resident care team members.
  • Attends and participates in community and resident care meetings.
  • Promotes a collaborative and positive working environment.
  • Communicates with residents' families and addresses their concerns.
  • Manages Behavior Expressions and overall wellness needs of Harmony Square residents.
  • Carries out other duties as assigned.

Qualifications/Skills/Educational Requirements

  • Current valid and unencumbered RN or LPN license in the state where the community is located.
  • Areas of study: Nursing.
  • Experience: 1-2 years required, 3+ years preferred.
  • Special Certificates: Current CPR certification.
  • Skills: Strong organizational and time management, effective verbal and written communication, ability to work flexible schedules including evenings, nights, weekends, and acting as Manager-on-Duty.
