Responsibilities
- Develop, support, and manage clinical pharmacy-related programs and services across all departments of Mount Sinai Medical Center.
- Manage and monitor the hospital drug formulary: complete drug class reviews, minimize non-formulary procurements, suggest appropriate, cost-effective therapeutic alternatives to medical staff, and implement automatic therapeutic substitutions when appropriate.
- Identify, design, analyze, and report drug utilization reviews to promote rational and safe drug therapies and determine action plans based on results.
- Train and participate in Disaster Team drills and real-event responses as assigned. Participate in quality and regulatory reviews and audits.
- Work with Pharmacy leadership to identify and develop processes to meet or exceed all regulatory standards.
- Act as manager in charge as assigned. Effectively and proactively intervene in conflicts between pharmacy and medical staff regarding drug therapy issue resolutions.
- Work with Pharmacy leadership to establish goals and objectives of clinical pharmacy programs and be responsible for implementation and monitoring within budgetary targets.
- Participate in various nursing and medical staff committees as necessary. Coordinate all activities related to the Pharmacy and Therapeutics Committee, including the publication of its newsletter.
- Support the coordination of the pharmacy review of medication orders in physician order sets, protocols, or other evidence-based medication use guidelines across the organization.
- In conjunction with the Pharmacy Management team, be responsible for the development and review of clinical pharmacy policies and procedures.
- Maximize the area's impact by ensuring adequate staffing levels and the most efficient organization and use of resources to support all clinical programs.
- Develop and actively participate in training and mentorship programs for pharmacists, pharmacy students, residents, technicians, nurses, and physicians as it relates to clinical pharmacy services.
- Conduct ongoing analysis and reporting of clinical pharmacy interventions to ensure accurate, high-quality, and appropriate documentation is being completed.
- Build effective relationships with other healthcare professionals and departments within the medical center.
- Define the team's target accountabilities and the guidelines and practices to accomplish them.
- Ensure the implementation and completion of the team's regular and special projects as assigned.
- Complete Performance Evaluations with timely and constructive feedback for the assigned team.
- Follow the hospital's policies and procedures regarding Human Resources Management.
- Ensure new employees complete general and department-specific orientation as set forth in the hospital's policies and procedures. Document all personnel actions and submit them immediately for review.
- Guide and direct team efforts. Ensure the team has the necessary resources to perform as expected.
- Analyze the team's outcomes with ongoing assessment of established goals. Identify areas of opportunity and respective action plans.
- Ensure all procedures are carried out in accordance with company policy.
- Ensure customers receive standards of service in line with the policy and strategy.
- Support the day-to-day activities of the staff.
- Follow the National Patient Safety goals as required:
- Improve accuracy of patient identification with at least two identifiers (excluding the patient's room number).
- Reduce the risk of health care-associated infections.
- Reduce the risk of patient harm resulting from falls.
- Follow Patient Safety goals as established by the department; support effective responses to adverse and sentinel events; provide an ongoing proactive approach to reduce risk; integrate patient safety as a priority into new and existing processes.
- Be available to participate as a member of either Team A or Team B of the Hurricane Activation Plan, as assigned.
- Report incidents to the Director and/or Risk Management department within the required timeframe in accordance with medical center policy.
- Comply with current Hospital security and safety policies.
- Perform all other tasks as assigned.
Qualifications
- License/Registration/Certification: Licensed as a Pharmacist in the State of Florida.
- Education: Graduate from an accredited School of Pharmacy. Master's Degree in Business or Healthcare Administration-related fields preferred.
- Experience: Eight+ years in a Pharmacy Management comparable role.
