Associate Dental Care and Population Health Management Outreach Specialist

About the role
Supports care coordination, outreach, education, and quality-related activities for Healthy Kids Dental (HKD), Healthy Michigan Plan (HMP)/adult Medicaid health plan partners and other tristate Medicaid or Medicare Advantage plans. Assists members, families, providers, and internal teams with access to services, documentation, communications, and program support.
Responsibilities
- Responds to routine inquiries from members, families, providers, and other stakeholders, connecting them to available services, resources, and program information for Medicaid and/or Medicare programs.
- Assists with coordinating service plans and follow-up activities by gathering information, tracking member needs, and supporting care goals under guidance from more experienced staff.
- Enters and maintains accurate documentation of member interactions, assessments, service plan updates, outreach activities, and outcomes in designated systems and databases.
- Provides standard education and support to members and stakeholders through outreach calls, care coordination activities, community events, virtual engagement, and other approved communication channels.
- Updates and organizes policies, procedures, work instructions, and other operational documentation as directed to support departmental activities.
- Supports relationships with community organizations, professional associations, and other stakeholders by coordinating information, scheduling activities, and assisting with outreach and engagement efforts.
- Assists with preparing content, materials, and communications that promote awareness, education, service utilization, and program goals.
- Supports communication campaigns and outreach initiatives by assisting with content preparation, stakeholder communications, scheduling, reporting, and basic follow-up on results.
- Serves as a support resource for internal and external partners by assisting with quality improvement activities, member engagement efforts, care coordination tasks, program support, and performance improvement projects.
- Identifies routine issues or trends and shares observations with leadership or experienced team members to support improvements in member engagement, access to services, utilization, and outcomes.
- Provides administrative and operational support to departmental teams by preparing routine reports, communications, documentation, and responses to requests from internal stakeholders and business partners.
- Performs other related assigned duties as necessary to complete the responsibilities described above.
Requirements
- Bachelor’s degree in dental hygiene, healthcare administration, public health, or a related field.
- One year of experience as a dental hygienist or supporting patient education, community or provider outreach, public health, dental education, care coordination, or related program activities.
- Community Health Worker (CHW) certification and/or a Registered Dental Hygienist (RDH) license issued by the State of Michigan, Indiana, or Ohio is preferred.
- Ability to speak Spanish or Arabic is preferred.
- Basic knowledge of dental terminology, procedure codes, and the ability to review radiographs, electronic images, and photos with guidance.
- Knowledge of word processing, spreadsheet, and database applications.
- Verbal and written communication skills.
- Basic analytical skills and the ability to provide clear information to members and stakeholders.
- Ability to follow established procedures, escalate issues appropriately, work independently on routine tasks and as part of a team, and solve routine problems using established guidelines.
