Associate Care Advocate - Care Delivery

Wider Circle is hiring an entry-level Associate Care Advocate with a clear path to move into a more senior role. In this position, you will call patients by phone to learn about the challenges affecting their health—such as trouble accessing food, housing, or medical appointments—and connect them with relevant services while maintaining clear, accurate records of each call.

Responsibilities
- Screen Patients and Manage Intake: Call patients and ask health-related questions using a set script (calls typically last 15–20 minutes). Your goal is to make patients feel comfortable so they share honest answers.
- Connect Patients to Resources: Ask questions to identify each patient’s needs, then share resources and send referrals to health and mental health services.
- Keep Accurate Records: Document patient responses in an Electronic Health Record (EHR) during each call. Review and finalize notes by the end of each workday, ensuring compliance with federal privacy rules (HIPAA) and billing standards. Collect and record patient consent at the start of each call.
- Manage Your Schedule: Maintain your own daily calendar, send reminder calls to reduce missed appointments, and meet weekly goals. Schedule follow-up calls for patients and allocate time between appointments to complete notes.
Requirements
- Education: High school diploma or GED required. A degree in Human Services, Public Health, or a related field is a plus.
- Languages: Bilingual in English and Spanish (or another language) is strongly preferred.
- Certification: Community Health Worker (CHW) certificate preferred. If you do not have one, you must be willing to earn it by completing training provided by Wider Circle and/or a state certification application.
- Experience: Experience in healthcare, social services, or community work is helpful but not required. You must feel comfortable speaking with diverse individuals by phone. Full training is provided.
- Tech Skills: Comfortable using websites and apps to enter and track data. Able to adapt in a fast-paced work environment.
- Self-Directed: Able to manage your own tasks, stay on top of notes, and meet goals without close supervision.
- Training: Willing to complete our onboarding program, including call practice, documentation exercises, and role-play. You must pass a final test with a score of 80% or higher.
- Background Check: Willing to complete a drug test and background check before starting.
Physical requirements: Ability to sit, stand, and walk as needed. Must have adequate vision and hearing to speak with patients by phone and in person. Must be able to type and use multiple computer programs simultaneously while talking. Requires a strong internet connection and a quiet, private space for patient calls.
