Assisted Living - Health Services Coordinator
Essential Functions
- Develops and maintains strong, positive working relationships with care associates, physicians, residents and families.
- Ensures accurate and timely completion of assessments and guides daily resident care and services in accordance with physician orders, care plans, standards of care and practice and resident-centered goals and preferences.
- Maintain environments that promote feelings of safety, security, and overall well-being for residents within all areas of the community.
- Maintain a positive social model resulting in a homelike environment where residents’ needs are exceeded and all associates participate in all-day programming and engagement with residents.
Responsibilities
- Oversee the care and operations of the Assisted Living community.
- Create a positive workplace for all associates, and recruit and retain an exceptional care team.
- Supervise, direct, lead, and nurture personal/professional growth of all Assisted Living associates.
- Train associates and model expectation for all-day purposeful, meaningful engagement with residents, as well as train and model expectations for resident’s successful Activities of Daily Living (ADL) and dining experience.
- Oversight of all aspects of Assisted Living, including care plan meetings, family and associate satisfaction, quality assurance and performance improvement, performance reviews, assists in coordinating resident services, and assists with Leasing and Marketing efforts.
Requirements
As a minimum requirement, qualified candidates must have a valid Ohio LPN certification. Experience in senior living and training in dementia care is strongly preferred. Able to and demonstrated success in effectively leading, inspiring and supervising a diverse employee work group; passionate, inclusive leader and manager who believes that their associates are their #1 priority. Excellent written and verbal communication skills and professional demeanor. Has sufficient computer skills; is tech-savvy or a very quick learner of computer skills to ensure efficient performance with all computer- and technology-based systems; experience with EHR interface preferred.
