About the Role

The Assistant Director of Wellness uses specialized knowledge to ensure Residents’ care and management needs are adequately and correctly met. This role assists the Director of Wellness as a liaison between Residents, their legal representatives, families, administrative staff, and community-based service providers.

Schedule

Tuesday–Saturday, 9:00 AM–5:30 PM

Responsibilities

  • Direct supervision of the Wellness and Reflections Unit.
  • Motivate, supervise, and provide in-service training/education for Resident Service Aides.
  • Overall supervision of the Resident Services Department in the absence of the Director of Wellness, including cross-coverage for Wellness and Reflections.
  • Implement emergency procedures as outlined in the Emergency and Disaster Plan.
  • Respond to requests for information and other activities as directed by the Executive Director, Director of Wellness, or Vice President of Clinical Services.
  • Ensure 24/7 operation of the Wellness/Reflections Unit when the Director of Wellness is unavailable, including:
    • Monitoring proper scheduling and staffing in accordance with company policy and State Health Department regulations.
    • Collaborating to secure adequate coverage as needed.
    • Being available and responsive for immediate consultation with staff, management, Residents’ families, or providers.
  • Maintain ongoing infection control surveillance, OSHA standards, universal precautions, and sanitation in the Wellness Office and Reflections Unit.
  • Ensure accuracy of Resident records and documentation in compliance with company policies and State Department of Health regulations.
  • Review electronic health records dashboard throughout shifts to ensure compliance.
  • Oversee the medication management system to adhere to State and local Department of Health laws, regulations, and company policies, including:
    • Conducting thorough reviews of Medication Administration Records, including “chart to cart” audits.
    • Addressing findings and developing training strategies to correct issues.
  • Follow company policies and State Department of Health regulations for incident reporting and notifications.
  • Maintain active community and professional relationships to promote the company positively.
  • Develop contacts with community agencies serving senior citizens and clergy, and refer Residents as appropriate.
  • Liaise with nursing homes, hospitals, and assisted living facilities in the community.
  • Conduct daily observational walk-throughs of the Main Dining Room and Reflections Dining Room to ensure Residents are appropriately groomed and dressed.
  • Monitor and maintain documentation on the Emergency Response System for compliance.
  • Act as a Resident advocate by:
    • Visiting with Residents to evaluate concerns.
    • Referring complaints to the Director of Wellness or Executive Director for resolution.
  • Oversee the admission process, including:
    • Conducting pre-move-in evaluations.
    • Reviewing and ensuring accuracy of Residents’ physician evaluations.
    • Conducting or delegating Initial Level of Care Evaluations prior to admission and follow-ups as required.
  • Provide initial and ongoing orientation for new Residents and introduce them to others.
  • Encourage Resident participation in social activities.
  • Inform the Executive Director, Director of Wellness, and Department Heads of Residents’ well-being and ability to live in the Assisted Living setting.
  • Communicate changes in Resident status to their representatives and appropriate administrative staff if delegated.
  • Monitor Wellness and Reflections Communication Logs daily for unusual occurrences or changes in Resident status.
  • Ensure compliance with corporate policies, procedures, and State Department of Health regulations for Assisted Living Residences and Special Needs Programs.
  • Supervise Wellness and Reflections Unit staff to ensure Residents receive appropriate care and services.
  • Coordinate care with Department Heads, Residents’ physicians, and third-party providers, including:
    • Assisting with referrals to pertinent agencies (e.g., Long Term Care insurance, nursing homes, social services).
    • Facilitating transfers and discharges to/from hospitals, skilled nursing facilities, and rehab centers.
    • Conducting off-site visits as requested.
    • Liaising with Residents’ personal physicians and other healthcare providers.
  • Develop and maintain accurate Resident records in compliance with company policy and regulatory rules, including:
    • Collaborating on Resident service/care plans with multidisciplinary teams, healthcare providers, and families.
    • Reviewing and revising service plans every six months or when Resident status changes.
    • Documenting social, recreational, physical, emotional, and mental/behavioral health findings, interventions, and outcomes.
    • Reviewing visiting physician notes and implementing prescribed changes.
    • Ensuring annual physician evaluations are completed.
    • Overseeing and documenting reviews of Residents self-administering medications to ensure continued competency.
    • Conducting skin observations and documenting findings for compliance.
  • Staff education, development, supervision, and evaluation, including:
    • Interviewing, recommending for hire, training, supervising, evaluating, coaching, and disciplining staff as requested.
    • Supervising care staff and scheduling in-services to improve care quality.
    • Following company in-service schedules to meet State Department of Health requirements.
    • Ensuring Medication Aides comply with training and in-service requirements.
    • Conducting periodic health seminars for Residents and families in collaboration with the Director of Recreation.
    • Overseeing the staff audit process, including:
      • Monitoring completion of audits (e.g., ERS, Controlled Substances, Medication Administration Records).
      • Reporting findings to the Director of Wellness.
      • Assisting with plans of correction and follow-ups to ensure compliance.
    • Following workplace injury procedures, including reporting and corrective actions.
  • Perform other duties as assigned by the Director of Wellness, Executive Director, or Vice President of Clinical Services.
