About the role

The Assistant Director of Social Services position at CareOne’s Newton facility is seeking an experienced professional with a Master's Degree in Social Work or a related field. This role requires a minimum of three years of social work experience, with long-term care/skilled nursing experience preferred. The Assistant Director oversees the Social Service department, providing support services, direct services, advocacy, counseling, and more.

Responsibilities

  • Plan and direct a comprehensive social service program supporting residents and families.
  • Participate in interdisciplinary team meetings to address the needs of residents.
  • Provide therapeutic interventions and counseling to help residents cope with their conditions and feelings about institutionalization.
  • Maintain detailed documentation in resident medical records.
  • Develop and implement social service components of the resident's plan of care.
  • Coordinate interdisciplinary care planning conferences and ensure resident/family involvement.
  • Act as a liaison between residents, families, and external agencies to uphold resident rights.
  • Conduct family meetings and discharge planning processes.
  • Ensure adherence to safety protocols and fire/disaster plans.
  • Manage admissions and coordinate with referral sources.
  • Facilitate tours for interested parties and verify insurance coverage.

Requirements

  • Master's Degree in Social Work or a human services field.
  • Current CSW or LSW license in the state of operations.
  • Three or more years of social work experience.
  • Experience in long-term care/skilled nursing settings.
