Assistant Clinical Manager (LVN) - Home Health

Responsibilities

• Assist the Clinical Manager in managing the clinical department, including staffing, scheduling, and budgeting.
• Schedules and appropriately documents patients schedules in a timely manner.
• Participates in coordinating care with management and patient interdisciplinary team.
• Provides effective communication to patients, staff members, other healthcare professionals, and referral sources.
• Responds to Agency and patient needs in a professional and creative manner. Demonstrates commitment, professional growth, and competency.
• Promotes the Agency philosophy and administrative policies to ensure quality of care.
• Reaches out to and follows up with doctor offices in regard to patient care.
• Manages the flow of patient care.
• Review and process clinical reviews.
• Communicates with the pre-authorization team.
• Discharges patients from care under the direction of the Clinical Manager.
• Send lab referrals to the approved destination.

Requirements

• Current and valid LVN license in the state of Texas.
• Minimum of 3 years of clinical experience.
• Previous experience in a leadership or management role.
• Excellent communication and interpersonal skills.
• Strong analytical and problem-solving skills.
• Ability to work collaboratively with other departments and stakeholders.
• Valid LVN License.
• Experience with Kinnser/Wellsky EMR.
